The medical Action Plans are in a yellow binder on the back desk/work area behind the dividing partition, near the sink.
When new ones are received, the month and year should be noted at the top, then they are added in year order 1-8.
If replacing an existing Action Plan, check carefully that the correct (new) one is going into the folder. Any old plans must be securely disposed of i.e. ‘blue box’.
Note that these must not be displayed where they can be seen by anyone but staff.
